• Vineland Adaptive Behavior Scales (VABS-3) $250.00

• Wechsler Adult intelligence Scale (WAIS-IV) $750.00

• Childhood /autism rating Scale(CARS) and the Autism Diagnostic Observation Schedule both with the Autism Specialty Report $750.00

• Social History Interview  $150.00

* Currently we do not accept insurance but are reasonably priced and short wait time.

Consulting and testing services are scheduled upon receipt for payment,. We accept credit card or check payments.

All testing is done at Self-Initiated Living Options, Inc (SILO) 3253 Route, 112, Bldg. 10, Medford, NY 11763.

Hours vary depending on the family’s availability. Appointments generally scheduled in evening or on Saturday.
